IT Website Form and Survey Creation
The Web Development team can assist you with any type of form and survey need, whether it's a PDF or an online fill able form. We will help you from the design phase until the data retrieval process.
Minimum of 4 weeks lead time must be given prior to survey running.
Currently we offer 3 types of online forms;- PDF - printable and/or fillable
- Online forms like suggestions and comment boxes
- Surveys - added to your site or in a secure setting like my`VILLE or myMU
- Send an email for every submission
- Export results to Excel
- Export results to SAS for statistical analysis
- Database availability for those with Brio access
- Gather form/survey requirements from client
- Suggest type of form (PDF or web form)
- Determine data collection method
- Design and layout - integration into existing site or create an identity
- Development of form and processing
- Testing and review by client
- Form is moved to production and if necessary a date is set for data retrieval
